This certification is obtained through your insurance company. Photocopies are not accepted. The minimum insurance requirements are:

- Minimum uninsured motorist insurance (covers damages caused by an uninsured motorist and suffered by you): $20,000 for bodily injury per person, $40,000 for bodily injury or two or more people, and $15,000 for property damage.
- Personal Injury Protection (PIP) provides benefits for medical, hospital and disability up to $2,500 for all reasonable expenses arising out of an accident. You may also choose limited PIP, which excludes benefits for you and members of your family age 16 and over. Ask your insurance agent about this lower-cost alternative costs less if you already have medical and hospitalization insurance.
You may also have to provide proof of insurance if you change insurance companies. When your previous insurer informs the MVA of your policy cancellation, the MVA may notify you that proof of insurance is required from your new insurance carrier. Your new company does not automatically notify the MVA.
